Job Title: Guardianship Case Manager/Maryland Access Point -MAP Specialist

Employment Type: Full-time, Salaried Exempt Position/40+ hours per week

Monday–Friday | 40 hours per week

On-Call Rotation (1 Weekend/Month plus coverage as needed)

Position Overview:

The Washington County Commission on Aging, Inc., is seeking a dedicated and compassionate professional for a dual-role position serving as both a Guardianship Case Manager and MAP Specialist.

This full-time role is evenly split:

50% Guardianship Case Management – Providing advocacy, coordination of care, and oversight for individuals under public guardianship.

Key Responsibilities:

  • Make regular in-person visits to clients (at home, hospitals, or care facilities).
  • Monitor health, safety, and overall well-being.
  • Collaborate with healthcare providers, legal representatives, and service agencies.
  • Provide informed consent for medical treatments and living arrangements as needed.
  • Maintain accurate documentation and timely reporting for court and agency records.
  • Attend court hearings and team meetings as required.
  • Additionally, this position includes on-call responsibilities one full week inclusive of weekends, rotating holidays and provides coverage for time off for the full-time Guardianship Case Manager.

Key Responsibilities:

50% MAP Specialist – Offering person-centered assistance, resource navigation, and intake services for community members

  • Provide information, referral, and assistance to older adults, individuals with disabilities, caregivers, and families.
  • Conduct assessments to identify needs and connect individuals with appropriate    services and resources.
  • Assist with benefits, programs, and community resources, including Medicare, Medicaid, and other available supports.
  • Coordinate referrals and follow up with clients to ensure services are accessed and needs are addressed.
  • Maintain accurate and timely client records, documentation, and required program reports.
  • Collaborate with community partners and WCCOA programs to coordinate services.
  • Participate in outreach, education, and community events to promote Maryland Access Point services.

Qualifications:

  • Required: Associate’s Degree in Human Services or related field
  • Preferred: Bachelor’s Degree in Human Services, Social Work, or a related field
  • Case management experience required
  • Strong documentation and computer skills essential

· Excellent communication and interpersonal abilities

· Demonstrated ability to learn quickly and work independently in a fast-paced environment

· Must possess a valid driver’s license and have reliable transportation with current auto

   insurance

· Must pass a criminal background check, physical and drug screen

· Vaccines are required for this position

Compensation & Benefits:

  • Salary Range: $41,600

Full-time –non-exempt position; 37.5 hours per week, Monday – Friday 8:30 am – 4:30 pm

This entry-level position in Hagerstown, MD offers growth, change, and hands-on learning while supporting senior clients as a Maryland Access Point (MAP). This position provides assistance to individuals and their families to navigate and access home & community-based services/resources in an effort to adopt healthy behaviors through goal setting and advocacy.

Exceptional organization and interpersonal skills, with attention to detail required; strong oral/written communication skills are a must.  Must be able to identify and use resources, have initiative, be self-directed, and have the capacity to work independently.   Must be flexible and possess the ability to multitask in a fast-paced, changing environment. Ability and willingness to provide emotional support, encouragement, and motivation to individuals as well as the ability to initiate and maintain positive working relationships with staff and other organizations. Salary:  Hourly $16.50

Requirements:

  • High School Diploma/GED General Studies. Some college credits preferred. 
  • Organizational skills, computer skills (internet, email, and Microsoft Office programs), creativity, and flexibility required.
  • Basic computer experience is required.
  • Requires the ability to travel throughout Washington County using a personal vehicle.
  • Must have a valid driver’s license with insurance.
  • Background check required.
  • Physical, Immunization check, and drug screen required.
  • Annual Flu Vaccination required
  • Must be able to lift/push/pull up to 50lbs.

  • Hourly Rate: $17.50
