Washington County has the highest death rate from diabetes in all of Maryland and nearly half of diabetes cases occur in people over fifty-five. The COA's evidence-based Diabetes Case Management Program addresses this health care crisis. In order to ensure that diabetic seniors receive essential care, nurses make in-home visits to assist low-income, disabled seniors with regular checks for blood glucose levels, blood pressure, medication monitoring and nutritional compliance. This approach allows for frequent examinations of feet for neuropathy and wounds for healing. It differs from traditional diabetes management by bringing the service to the client, a method that will reach those who lack access to transportation or have limited physical mobility. With the support of trained nurses, clients will improve their own compliance, reduce diabetic complications and increase health.
