DOCUMENTATION REQUIRED FOR BENEFITS

Below is a checklist of possible items needed to prove eligibility for the program for which you are applying. Be prepared to show proof of those that apply to your situation:

 

Alimony Papers
Annuities – copy of annuity & last 3 statements
Birth Certificate/Proof of Citizenship
Burial or Funeral Account (fund or plan statement)
Checking Account Statement (last 3 months)
Court Papers specific to benefits requested
Cut off notice for utilities
Death Certificate
Divorce/Separation Papers/Child Custody
Eviction notice
Health Insurance Cards and/or Independence Cards (NOT your Medicare card)
Homeowner – copy of deed, property tax bill or mortgage statement
Income Tax return from previous year
IRA or Keogh (last statement)
Lawful Permanent Resident - form 1-94 or form from Dept. of Homeland Security
Lease – copy of lease if you rent (with landlord’s name, address & phone)
Life Insurance Policy (copy of original policy)
Picture ID for all adult household members
Power of Attorney (copy)
Proof of Disability
Proof of Income (past 3 months)
Child Support
Civil Service Annuity Award Letter
Disability
Interest Income
Mortgage contract for rental or business (you are lender & receiving $$)
Pay stubs, wage verification form (W-2) or proof of self-employment income
Rental/Lease income statements for property you rent/lease to someone else
Retirement/Pension of gross income (before taxes, etc. are deducted)
Social Security Award letter
TCA (temporary cash assistance)
TDAP (temporary disability assistance program)
Unemployment
Veterans Administration Award Letter
Workers Comp
Savings Account Statement – showing balance at the 1st of this month
Social Security Cards for all household members
Stocks, Bonds, 401-Ks (statement for last 3 months)
Trust Fund document (3 recent statements for those you have had in last 6 mos.)
Whole Life Insurance (cash value statement from insurance carrier)
