Maryland Access Point (MAP) is a new initiative that links
older persons and adults with disabilities from all income
levels with local health and support services. MAP was
developed by a partnership of community service organizations
on behalf of Washington County residents. The primary contact
for this program is the Washington County Commission on
Aging. MAP assists Washington County residents by:
- Creating a single point-of-entry to access resources for
lifelong independence
- Accessing the financial and eligibility processes for local,
state, and federal programs
- Offering a “No Wrong Door” connection to
appropriate agencies providing benefits or services
Washington County MAP serves:
- Persons
over age 50
- Adults with disabilities over age 18
- Family members and other caregivers
- Service providers and agency staff
- Adults who wish to plan for their own potential long-term
care needs
Programs and Services
The Washington County Commission on Aging is partnering
with the Washington County Mental Health Authority, Washington
County Health Department, Resources for Independence,
Inc., Washington County Community Action Council, Washington
County Department of Social Services and other community
agencies and resources to provide the following programs
and services:
- Aging in place
- Benefits counseling
- Care giving and in-home services
- Home modifications
- Housing options
- Legal services
- Long term care insurance information
- Medicare and health insurance
- Prescription assistance
- Transportation
- Volunteer opportunities
- Workforce options
- and much more
